What are you feeding (full diet routine please)?
I think there's a deficiency and that is how my old girl started with hex.
The white mark at the beginning of the lateral line is a symptom and the fin marks as well for some reason.
She did recover and went back to full weight.

Note all the white marks.

I would medicate the tank, yes.
I'd also feed more meaty protein, probably on a daily basis. Mine loved brine shrimp, bloodworm weekly is ok (don't feed every day), prawns are good now and again (monthly/every other month), snails are one of the best foods for them.
According to Tim, they also like cat food (tinned with the fatty bits taken out)........but I never tried it.
White fish is ok as well. Mine ate coley when we had it (I was quite glad as I thought it was a bit boring).

In the pic she'd actually started to get marks over her mouth as well, but you can't see them.

Beefheart never thought of that for loaches. I think iv maybe concentrated too much on veg as aposed to meaty foods. Il try Ians advise with the meds and also with meatier foods added. thanks to everyone for there advice and input.
